  • Stable office staff that is well compensated.  
  • What are the volumes like:
    • Average day at the Outpatient Surgery Center (ASC) is 15-20 cases per day (all cases, not just GS)
    • Main OR is probably 20-30 cases per day (all cases, not just GS)
    • General surgeons are doing anywhere from 200-400 cases in the hospital and 250-550 cases in the surgery center a year (includes GI work)
    • One physician doing vein work in the office and the second physician does wound care in the hospital’s wound care clinic.
  • Office hours:
    • 8a-4:30p
  • Call coverage:
    • 1 in 5 weekends (One provider will continue to do call)
  • Payor mix:
    • Strong payor mix-45% Medicare, 40% Commercial, 10% Medicaid, 5% Self Pay (I need to double check, commercial is generally much higher here)
  • Patients per day:
    • 20-25 in office
  • Billings/collections:
    • 1st physician at MGMA 90th percentile ($2,850,000/$925,000)
    • 2nd physician is at the median for GS cases ($1,390,000/$485,000) and his wound care work income puts him in the 75th percentile
    • 3rd physician started slowing down last year and retired. ($1,500,000/$455,000)
  • Who determines the schedule:
    • Surgeon block schedule design. Office schedules in the block
    • Patient demographics
    • Full spectrum 
  • What does an average day look like for the GS:
    • Office Monday-Thursday AM, Mon PM and Wednesday at 3or 4 for vein procedures.  
    • Surgery all day Friday and Tuesday-Thursday afternoon.
  • Procedures being done:
  • All general surgery procedures
  • Main OR:
  • All types of general surgery cases including acute abdominal cases, peripheral vascular, laparoscopic procedures of all types. Plastic surgeon on staff who works with GS for breast reconstructions for cancer patients.
  • 1 Da Vinci
  • 6 operating rooms
  • Sentinel node machine.
  • Anesthesia is very adept at different types of pain blocks for post-op pain control.  Hospital employed.
  • Blocks from 7:30 to 3:00, with one running to 4:00
  • Currently pursuing trauma level 3 designations.
  • GI Lab:
  • 2 GI suites, equipped with Olympus GI 190 scopes
  • EMR Provation documentation systems
  • Ambulatory Surgery Center (ASC):
  • 4 operating rooms equipped with Stryker endoscopy equipment
  • 2 GI suites, equipped with Olympus GI 190 scopes
  • Don’t do acute general surgery there, just planned Outpatient procedures
  • We do all of the main general surgery cases, except for bowel resections
  • Staff is employed by a management group, with doctors partnered with the hospital, dedicated CRNAs (hospital employed)
  • Blocks are from 8:00 to 4:00 Monday-Friday
  • C-Arm imaging, ultrasound machines,
  • Wound care opportunity (potential)
  • Compensation is personal production minus expenses
  • Group equally splits fixed expenses (rent, staff and staff benefits, etc)
  • Individual expenses (malpractice, CME, retirement, specialized supplies, etc)
  • Very little practice buy-in
  • Opportunity for surgery center buy-in after one year (would result in an additional 100k income)
  • Trauma Call: $1,000 weekday, $1,500 weekend
  • Hospital provides - year guarantee ($350k-$400k)
  • Very busy practice with plenty of patient volume.  One physician will be dialing back his productivity once new provider takes his place.

  • No (production-expense model). They generally take off 4-6+ weeks.
