Overview The physician functions as a health care provider within skilled nursing facilities (SNFs), sub-acute rehab and long term care facilities (LTCs) for the purpose of providing medical management and culturally sensitive care to patients or residents. Provides assessment and consultation with patients. Works with Landmark Connect, Medical Director, Facility, Director of Nursing and Administrator in the development and/or implementation of medical management policy, clinical protocols, and quality management programs. Dedicated to improving the health and well-being of the patients. The practice will involve care and management of patients in these settings. Care will include performing admission H&Ps, plan of care day-to-day management of patients and discharge planning. Responsibilities Practices in accordance with the respective state laws and regulations Must possess the ability to actively identify new areas for learning; regularly create and take advantage of learning opportunities, and use newly gained knowledge and skill on the job and learn through their application. Demonstrate ownership and responsiveness in their customer service approach to patient care and contributes to an atmosphere of teamwork within their group. Determines patient acuity after thorough assessment of the patient and review of the chart Understands and complies with long-term care policies and procedures, including those that regulate nursing facilities and assisted living communities. Establishes and maintains effective relationships with patients and their legal representatives, facility staff, and other collaborating physicians/providers. Works with the facility medical records department to schedule patients for required visits. Must maintain quality, operational, clinical and productivity goals as established by the leadership team. Works with Facility and quality staff to identify areas of opportunities to develop plans to increase quality to positively affect the facilities STAR ratings Following the visit promptly documents and communicates any pertinent information for the facility staff Completes required documentation in a timely manner Performs an initial comprehensive assessment on all new patients. Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers/acute admissions. Consults with hospital physicians following notification of member transfer. Collaborates with facility staff to enable medical necessity determinations for requested medical services. Educates members, families, and/or their legal representatives in disease processes affecting members and ways to manage them effectively, as well as to promote wellness. Provides counsel, support and education to facility staff. Supports FCMs in developing and providing in-service educational programs to facility staff. Participates in management meetings that include, but may not be limited to: Weekly case conference reviews Monthly staff meetings Schedule Joint Operating Committee meetings Quarterly Quality Improvement Committee meetings Continuing education/journal club programs Behavior health meetings Family meetings Implements quality initiatives as directed by the medical director or Chief Clinical Officer to ensure the highest standards of care and to promote the improvement of care management and delivery. On-call for caseload during business hours and assigned after hour management of the facility. Keeps current on relevant medical and nursing research, technology, and related issues by attending continuing education courses, professional meetings and journal reviews. Perform related duties as necessary and other duties as assigned. Qualifications Board Certified / Board Eligible Physician, preferably Internal Medicine, Family Medicine, or Emergency Medicine Doctor of Medicine M.D. or Doctor of Osteopathy D.O. from an accredited educational institution Knowledge of long-term care, community resources and cost-effective alternatives regarding patient care delivery systems Knowledge of CPT, ICD-10 and HCPC codes Knowledge of clinical standards of care Awareness about UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful Eagerness to work with a diverse and multidisciplinary team Certificates, Licenses, Registrations: Current active, unrestricted MD or DO license Current DEA registration Current unrestricted state driver’s license
